What Is a Deep Pore Cleansing Facial? Extractions, Results and Cost

Esthetician performing careful extractions during a deep pore cleansing facial at NY Healthy Skin Care in Midtown Manhattan

A deep pore cleansing facial is a professional treatment that softens the skin with steam, exfoliates it, then manually clears blackheads and congestion from the pores before calming everything down with a mask. It takes 50 to 75 minutes, costs between $90 and $200 in most US cities, and works best as a short series rather than a one-off.

If you have blackheads on your nose, chin or forehead that no strip, scrub or cleanser has ever shifted, this is the treatment built for exactly that problem. Here is what actually happens in the room, whether it hurts, and how many sessions you genuinely need.

The short version

  • What it is: cleanse → steam → exfoliate → extract → mask → protect
  • How long: 50–75 minutes
  • Does it hurt: pressure and pinching, not sharp pain
  • How often: every 4–6 weeks while clearing, then quarterly
  • Results: visible after one, meaningful after three to six

What a deep pore cleansing facial actually is

A basic facial cleans the surface. A deep pore cleansing facial empties the pore.

That difference comes down to one step: manual extraction. Everything before it exists to make extraction possible and safe, and everything after it exists to calm the skin down.

What causes the congestion in the first place is straightforward. Sebaceous glands produce sebum; when sebum mixes with dead skin cells inside the follicle, it forms a plug. Exposed to air, that plug oxidises and turns dark — a blackhead, or open comedo. Closed under the surface, it stays white. No topical product removes an established plug reliably. It has to be softened and lifted out.

Step by step: what happens during the treatment

2. Double cleanseMakeup, sunscreen and surface oil removed5 min
3. SteamWarm steam softens the plugs and relaxes the follicle openings8–12 min
4. ExfoliationEnzyme or gentle acid loosens the dead cells holding the plug in10 min
5. ExtractionsCongestion cleared manually with cotton-wrapped fingers or a sterile loop10–20 min
6. Soothing maskClay, calming or hydrating mask depending on your skin10 min
7. FinishSerum, moisturiser and SPF5 min

Steps three and four are the ones people skip when they try this at home, and they are exactly why home attempts go wrong. Without steam and exfoliation, a plug does not want to come out — so people push harder, and pushing harder is what damages the follicle wall.

Extractions: does it hurt, and is it safe?

Honest answer: it is uncomfortable, not painful. Most people describe pressure and a brief pinch, strongest on the nose and the inner cheeks where skin is thin and nerve endings are dense. The forehead and chin are usually easy. Nothing about it should make you flinch or brace.

Two things make it worse than it needs to be: skin that was not properly softened first, and an esthetician working too fast.

What a good esthetician does

  • Softens the skin properly before touching a single pore
  • Works in short passes, moving on and coming back
  • Stops on a plug that will not release rather than forcing it
  • Uses cotton or a sterile loop, never bare fingernails
  • Cleans and calms as they go, not just at the end

What should not happen

  • Repeated hard pressure on the same spot
  • Extraction of inflamed, pustular or cystic lesions — these are squeezed at the cost of a scar
  • Bleeding beyond a pinpoint
  • You leaving with skin that feels raw rather than clean

If it hurts sharply, say so. A competent esthetician will adjust immediately.

Who it's for (and who should skip it)

Close-up of oily congested skin on the nose and chin, the skin type suited to a deep pore cleansing facial

This treatment is built for you if:

  • You have visible blackheads on the nose, chin or forehead
  • Your skin is oily or combination and congests reliably
  • Pores look enlarged and full rather than empty
  • You wear makeup or sunscreen daily and feel "clogged"
  • You get closed bumps under the surface that never come to a head

Wait or choose something else if:

  • You have active inflamed or cystic acne — extraction of inflamed lesions makes acne worse and scars. An acne facial with LED is the better route
  • You are on isotretinoin, or stopped less than six months ago
  • You used retinoids, acids or benzoyl peroxide in the last 3–5 days
  • You have active eczema, rosacea flare, cold sores or sunburn on the area
  • You had laser, waxing or a peel on the face within two weeks

Men get this treatment as often as women — thicker sebaceous glands and daily shaving make congestion along the jaw and nose common. We run a dedicated version at our Midtown East location; see men's skin care.

Results: what to expect, and how long it lasts

Clear refined skin with visibly emptier pores after a series of deep pore cleansing facials

Immediately after: pores visibly emptier, skin brighter, and some redness where extractions were done. That redness settles within two to six hours.

Next 24–72 hours: occasionally a small purge — congestion that was already forming coming to the surface faster. This is normal and short.

One to two weeks: the honest window for judging a single session. Texture is smoother, pores look smaller because they are empty rather than because they shrank.

After a series of three to six: this is where the real deep pore cleansing facial before and after difference shows. Congestion clears faster than it rebuilds, and skin stops cycling through the same blackheads.

Pores do not permanently shrink. Anyone promising that is selling something. What changes is how full they are — and an empty pore reflects light differently, which reads as smaller.

Results last four to six weeks on oily skin, longer on drier skin. That is the sebum cycle, not a failure of the treatment.

How much a deep pore cleansing facial costs

Calm reception area of a licensed Midtown Manhattan skincare studio where deep cleansing facials are booked
SettingTypical range
Chain or budget spa$65 – $95
Independent licensed studio$100 – $180
Medical spa or dermatology clinic$175 – $300
Major metro premium (NYC, LA, SF)25–50% above the above

At our Midtown Manhattan studio, the European Facial — deep cleansing with careful extractions — is $150 for 60 minutes. It is the treatment most first-time clients start with.

What actually drives price is time spent on extractions. A $70 facial that allots five minutes to extraction is not the same treatment as one that allots twenty. Ask how long extractions take before you book — it is the single most useful question, and it separates a real deep pore cleansing facial treatment from a basic facial with a different name on the menu.

Why squeezing at home makes pores worse

Inflamed red mark left on the skin by squeezing a blackhead at home instead of professional extraction

Every esthetician sees the same three injuries, and all three come from the bathroom mirror.

Broken follicle walls. Pressure from the surface pushes contents sideways into the surrounding tissue instead of up and out. That triggers inflammation, which is how a blackhead becomes a red bump that lasts three weeks.

Post-inflammatory marks. Squeezing inflames; inflammation drives pigment. On medium and deep skin tones especially, a five-second squeeze can leave a dark mark that takes months to fade.

Scarring. Repeated trauma to the same pore is exactly how ice-pick scars form.

Pore strips are not a solution either. They remove the top of the plug and the sebaceous filaments that are supposed to be there, leaving the base behind to refill within days.

If you take one thing from this page: the difference between a professional extraction and squeezing at home is not skill with the fingers. It is the twenty minutes of steam and exfoliation that come first.

Aftercare: keeping pores clear between treatments

Salicylic acid cleanser and non-comedogenic moisturiser used as aftercare between deep cleansing facials

For 24 hours: no makeup, no gym, no sauna, no swimming, no picking. Cleanse gently, moisturise, and use SPF 30+.

For 3 days: no retinol, acids, scrubs or cleansing brushes.

Ongoing — the part that decides your next appointment:

  • A salicylic acid cleanser or leave-on, 2–3× a week. BHA is oil-soluble, so it gets inside the pore where AHAs cannot
  • Moisturise daily even if you are oily. Dehydrated skin overproduces sebum
  • Non-comedogenic sunscreen, every day
  • Change your pillowcase twice a week
  • Remove makeup and sunscreen properly at night — a single cleanse rarely does it
  • Rebook every 4–6 weeks while you are clearing, then quarterly to maintain

Between appointments, a lighter treatment like hydradermabrasion keeps pores clear without a full extraction session.

Frequently asked questions

Does a deep pore cleansing facial hurt?

It is uncomfortable rather than painful — pressure and a brief pinch, strongest on the nose. If it hurts sharply, the skin was not softened enough or the extraction is being rushed. Say something; a good esthetician will adjust straight away.

How long does a deep pore cleansing facial take?

Between 50 and 75 minutes. Extractions alone take 10 to 20 minutes of that, and how much time is given to extraction is what separates a genuine deep cleansing facial from a basic one.

How often should I get one?

Every four to six weeks while you are actively clearing congestion, then once a quarter to maintain. That matches how quickly sebum and dead skin rebuild a plug.

Will my skin break out afterwards?

Sometimes, briefly. Congestion that was already forming can surface faster in the first 24 to 72 hours. It should settle within a few days — a breakout that worsens after day three is worth mentioning to your esthetician.

Can I wear makeup after?

Wait 24 hours. Pores are open and makeup applied too soon is the easiest way to reclog what was just cleared.

Is it safe for sensitive skin?

Usually, with a shorter steam, an enzyme rather than an acid, and lighter extraction. Tell your esthetician before the treatment starts, not after. Active rosacea or eczema on the area means waiting.

What's the difference between this and a European facial?

At most studios, including ours, they are the same treatment — a European Facial is deep cleansing with careful extractions. The name varies by menu, not by method.

Do extractions make pores bigger?

No, when done correctly. Pore size is largely genetic; what changes is how full the pore is. Squeezing at home can stretch and damage a follicle permanently, which is a different thing entirely.

Can men get a deep pore cleansing facial?

Yes, and it is one of the treatments men benefit from most. Thicker sebaceous glands and daily shaving make congestion along the nose and jaw common.

How many sessions do I need to see a difference?

One session gives a visible result. Three to six, spaced four to six weeks apart, is where congestion starts clearing faster than it rebuilds — that is the meaningful change.

Can I do extractions at home?

Not safely. Without proper steam and exfoliation the plug does not release, so you push harder — and that is what breaks follicle walls, leaves dark marks and causes scarring. Pore strips only remove the top of the plug.

What should I do the day before?

Stop retinoids, acids and benzoyl peroxide three to five days beforehand. Do not exfoliate, wax or shave the area the same day. Arrive with clean skin if you can, and mention any medication you are on.
